Office Policies and Forms
Welcome to our medical office. Dr. Miller is committed to offering quality, time-intensive evaluation, diagnosis and treatment consistent with the principles of Osteopathic Medicine. This includes a comprehensive intake exam, an assessment of your current health status each time you visit, and hands-on treatment lasting 45-60 minutes designed to assist you in restoring your long-term health. Appointments
Your intake appointment is at least a 3 hour time slot reserved only for you and therefore we request an advance partial deposit when you schedule your first appointment. Please return your deposit with your completed intake forms below. If you need to change or cancel your appointment, please let us know immediately. Failure to give notice of at least 5 working days prior to an intake appointment may result in the loss of your deposit unless your time slot can be filled. Follow-up appointments are scheduled for 60 minutes. Kindly provide notice of cancellations or changes as soon as possible. Cancellations with less than 2 full business days notice will incur charges if your slot cannot be filled.
Payment
We ask for payment in full at the time of your visit. We accept personal checks. Credit cards are not currently accepted. As a courtesy we will prepare an insurance claim form for you to submit to your insurance company. Your insurance company may then reimburse you directly according to your benefit plan.
Insurance
In order to help you determine the extent to which your insurance policy will reimburse for Dr. Miller’s care, please consider the information below.
Osteopathic Medicine is considered a standard medical practice (not “alternative medicine”) and Osteopathic Manipulative Treatment (OMT) is a standard medical procedure (not “alternative”). As such, these services are generally recognized and reimbursed by medical insurance.
Dr. Miller has chosen to be an “out-of-network” medical provider. We recommend that you contact your insurance company to determine your coverage for medical services by out-of-network providers. We are sorry that we are not able to accept Medicare, Oregon Health Plan or Worker’s Compensation. Patients with Medicare may be treated at their own expense without insurance billing. Since Dr. Miller is not a member of any Worker’s Compensation panel, he cannot provide treatment for a Worker’s Compensation injury. Please contact the office if you have questions about your individual circumstances.

To protect your privacy, please do not send your forms by email. We prefer to receive your forms and deposit by standard USPS mail.
Preparing for your visit
Please allow up to 4 hours for your initial intake and 60 minutes for any subsequent visits. Wear loose-fitting and comfortable clothing such as sweat pants and t-shirts to allow ease of movement during treatment. Avoid jeans. As some of our patients have chemical sensitivities, we ask that you do not wear scented products to our office.
